VETERANS' SURVEYs

 

There are five areas of veterans' concerns and interests listed below.  Your personal identity is not required in answering these questions.  We ask that you submit a valid email address which will be verified in order that we may assure an accurate count in our data gathered.  Your email address will remain private and confidential.

It is essential to know how many veterans are sick from environmental exposures (and vaccines), with what kinds of symptoms and conditions, and the quality of care they are receiving by DoD/MoD and the VA.  The government is good for doing its own surveys and research, but seldom releases any information publicly on its findings unless it makes them look good.  Let's find the real numbers and gather information from ALL veterans from ALL eras-- not just accept the sampling of veterans that have been researched and reported by the government.

It is suggested to "log-in" for each survey form.  This allows you to come back at a later date to either make corrections or changes to your answers (i.e. changes in health conditions, etc.).  Help us to keep accurate numbers and survey results by taking time and careful consideration in all your answers.  There are blocks in each form to express any other things that weren't addressed in the survey form.  You may add to these blocks if you remember something later by using the log-in feature.

 

survey 1 - Personal History - Country of origin, branch of military and effects of exposures and experiences on veteran and family.  How has being sick affected the families and the careers of veterans?

Survey 2 - Kinds of Exposures Experienced (including vaccines) -   Kinds of exposures, as well as symptoms and conditions experienced by veterans.  What are real the numbers of veterans who were exposed to toxins?  How many veterans actually received adequate and thorough examinations upon return from duty and how have their medical records been documented?

survey 4 - Quality of Healthcare and Benefits -  Did you receive adequate information as to the care and benefits you are entitled to?  Do you know the laws and policies that govern the criteria for your diagnosis and claims ratings?
